Our Occupational Health Services support individuals with Intellectual and Developmental Disabilities (I/DD) in building the skills needed for successful employment. We offer personalized career planning, job placement assistance, vocational coaching, and ongoing support to ensure long-term success in meaningful employment.

Path to Employment Success: Our Step-by-Step Support

Phase 1

Personalized Career Planning and Assessments

  • Develop individualized service and career plans that reflect short-term and long-term goals

  • Conduct comprehensive assessments to uncover skills, preferences, and interests related to work

Phase 2

Access to Training and Support Services

  • Guide individuals in accessing training programs, further education, health and wellness resources, and recreational activities

  • Offer support to maximize independence and promote socialization

Phase 3

Customized Job Placement

  • Match individuals with the right job, work environment, and necessary natural supports

  • Focus on finding a position that aligns with their abilities and career goals

Phase 4

Ongoing Vocational Support and Coaching

  • Provide job coaching, travel training, and follow-up case management to ensure successful employment retention

  • Offer personalized support to secure and maintain jobs

Phase 5

Addressing Additional Barriers

  • Help individuals access services for barriers like housing, clothing, education, and counseling that may affect employment goals

Supported Transition Services

Our Supported Transition Services help individuals with special needs navigate life changes and successfully move into the next phase of their personal and professional journey.

Through customized support, we provide guidance, skill development, and resources tailored to each person’s goals, ensuring they have the tools needed for long-term success and independence.

 Services

Case Management

Each program participant is paired with a Case Manager who works closely with them to set and achieve both short-term and long-term employment goals. Case Managers play a vital role in guiding participants toward career advancement and educational opportunities, helping them build a path to economic self-sufficiency.

Case Management Services include:
Supporting personal well-being and creating financial asset development plans
Coordinating with partner organizations to access additional support services like legal aid, housing, healthcare, and higher education
Collaborating with Placement Specialists to prepare participants for successful job searches
